He spawns in the North Western quadrant of the room during the second phase if you failed to destroy the associated Amalgamation before the timer ran out. If you had you would be facing a weakened version of Cruor.
Once he and the 3 other praesul have spawned, it is suggested to kill Cruor 3rd. After first killing Umbra (Weakened) and then Glacies (Weakened).
