Made by using a Chisel (toolbelt works) on a Teleport to house tablet and selecting the Trollheim option. No experience is given for this action.
In order to have the ability to make this tablet, you must talk to Runvastr after completing Love Story. Ask him about when he was a miner and he will give you the Miner's diary. Take it to the Wise Old Man; he will tell you a short story and then give you the ability to make modify the Teleport to house tablet.
Unlike the Trollheim teleport spell, completion of Eadgar's Ruse is not necessary to use this tablet.
Tablet can be Right-clicked and modified again to repair it to a normal Teleport to house tablet or to convert it to another destination:
