He spawns in the South Western quadrant of the room during the second phase if you failed to destroy the associated Amalgamation before the timer ran out. If you had destroyed it, you would be facing a weaker version of Fumus.
Once he and the 3 other praesul have spawned, it it suggested to kill Fumus last. After first killing Umbra (Weakened), Glacies (Weakened), then Cruor(weakened).
