Fish at the "Fishing spot?" and accept the quest. This will begin a cutscene in which you encounter a broken nest and an egg from which a duckling soon hatches. You can choose to either comfort the Duckling or panic with it, either way it will follow you when you start looking for their parents. You will quickly notice your tiny stalker and decide to place it in your pocket to keep it safe from being lonely.
Note that despite putting her in your pocket, she isn't an item, so will not be in your inventory after the cutscene ends.

You will now need to follow a "trail" of Nest debris that starts near the bridge to the Wizards' Tower. Each time you pick up some debris, a new pile will appear. These can be found by the sparkles shimmering above the location and are marked by a red dot on the minimap. This trail will lead you all the way to Breadstix.
Note that as much as you are "picking up" the debris, none of it will appear in your inventory either.

Talk to Breadstix and ask what he's currently up to (chat option 2). He will tell you that he's trying to read a recipe scroll for a Duckspeak potion, but is having some issues understanding it. Ask what language the scroll is written in to continue the quest (chat option 2). After informing you that it is written in Linear Beak script, you will be given Breadstix's potion recipe.

If you don't already have your Duck food, make your way to Falador's East bank. South of the building to the West of the bank is a Sack of duck food, where you can get some. Once you have it, use it on your Vial of water to create a Duckspeak potion.

If you attempt to drink it immediately, you will think better of it before doing so and decide that you should talk to Breadstix first. Return to him, if you left the area, and talk to Breadstix about the potion (he will take back the recipe). You will confirm if he's ok with you drinking it to help the Duckling and he will encourage you to do so saying it's calling out to you. Given his blessing, drink the potion. You will feel dizzy and collapse... and wake up as a duck. When the cutscene ends you will find yourself in the entrance to Paddlington.
Note: You can use home teleports, but none of the other free teleports in this form (hilariously, this means you can use the home teleports while floating in the pond). If you leave by any means you can continue by returning to Breadstix and inspecting the Waddle Willow North of him. You don't need a new Duckspeak potion to do so, even though you will return to your natural form when you leave.

Go a little ways North and speak to Jemima. After she welcomes you to Paddlington, you will request a ticket in. She will inquire if you have completed the Inducktion - trials to fully access Paddington. With a bit of persuasion, you will agree to start the trail and Invigilatoad will appear.

With the cutscene completed, speak with Invigilatoad to start your Inducktion. Before you can begin though, you will be told the Duckling will need to wait for you in the duckcreche. After some convincing, she will agree to wait for you. Time to get started. The Invigilatoad will tell you to EAT THE PEAS.

When the conversation has finished, follow the paths to each of the peas (noted by red dots on the minimap) and eat each one. Once all are consumed, return to the Invigilatoad and talk to him about the next trial.

Now you will learn that the most important thing for a duck is to relax. You will question the simplicity of this endeavor, considering all of your other trials have had a bit more... peril to them. The Invigilatoad will comment that this may be more difficult than anticipated and you will be left to relax.

Climb the hill clockwise to the top and sit at the "relaxing spot" there. None of the other spots will be sufficiently relaxing. A meditation cutscene will begin and show you Paddlington. Once complete, you will find yourself back near Invigilatoad, Jemima, and the Duckling ready to progress.

When they finish congratulating you on completing your Inducktion, you will find yourself in front of the pond of Paddlington with the Duckling. Make your way to the Eastern floor button and press it.

Next, swim over to the West side and talk to Paddlington Badger to learn that the Duckling's parents are on the topmost level of Paddlington.

Then go to the South West side of the pond and press that one. After pressing the 2nd floor button (must be the South West one), you will try to convince the Duckling to wait there on that button while you go press the other one. To get her to stay, you will need to choose the "Something might happen?" chat option.

With her holding the South West button, make your way back to the East one (note you will still see her following) and you will both stand on the buttons at the same time. This will start a cutscene wherein light shines from the toad statue onto a turtle sculpture near the Eastern button.

Interact with the tree bud next to the turtle sculpture and you will now have a mouth full of sticky tree sap. Make your way to the toad statue to the North and interact with it. This will cause the statue to start blowing bubbles.

Interact with the statue again and you will use the bubbles to travel to the top level of Paddlington.

Near by, you will see the duckling's parents waddling around in a panic. Talk to either Duckling's mum or Duckling's dad to reunite the family.

When the conversation ends you will automatically return to Breadstix. Complete the dialog to finish up and claim your rewards.

Post Quest Rewards:
If you return to Paddlington after the quest (via the Monty the rubber duckie or the Waddle Willow) you can fish for peas in the pond. One fishing spot will appear at a time for you to fish from. Once you have collected 10 peas from it, that spot will vanish. Catching 30 peas in this manner completes the "I Want a New Duck" achievement, which is a requirement for obtaining the Master Quest Cape and unlocks the Waddles pet. The fishing spots will be available in the order in the image below and will be marked (as usual fishing spots are) with small bubbles
